C.J. Allen Upper Extremity & Hand Therapy maintains two Suffolk County locations, and our Smithtown office puts certified hand therapy within a short drive of most of the Smithtown township — including Nesconset, St. James, Kings Park, Head of the Harbor and Village of the Branch.

A Practice Devoted Entirely to the Upper Extremity

Hand therapy is not a service we offer alongside general rehabilitation — it is the whole of what we do. Every therapist here works exclusively on hands, wrists, elbows and shoulders, which means the volume of upper-extremity cases we see each week is far higher than a general outpatient clinic would encounter. That focus shows up directly in how quickly a problem is recognized and how precisely a splint is fabricated.

Certified Hand Therapists (CHT)

The CHT credential requires a minimum of three years of clinical experience, at least 4,000 hours of direct upper-extremity practice, and a comprehensive examination — then recertification every five years. It is a genuinely difficult credential to hold, and it is the clearest signal that the person treating your hand has spent years doing precisely that.

Conditions We Treat at the Smithtown Office

  • Carpal tunnel syndrome — conservative management, night splinting, nerve gliding, and post-release rehabilitation.
  • Trigger finger — splinting and tendon gliding to restore smooth, painless motion.
  • Wrist and hand fractures — protected mobilization, edema control and progressive strengthening after casting or fixation.
  • Tendon repairs and tendonitis — protocol-driven rehabilitation timed to the surgeon's repair.
  • Arthritis of the thumb, hand and wrist — joint protection, custom orthoses and activity modification.
  • Nerve compression and injury — sensory re-education and targeted strengthening.
  • Post-surgical rehabilitation — coordinated directly with your surgeon's protocol.

Custom Splinting, Fabricated On Site

Custom orthoses are made here in the clinic, molded to your hand rather than pulled off a shelf. For conditions like trigger finger, thumb arthritis or a healing tendon repair, fit is the difference between an effective splint and one that gets left in a drawer. Because we fabricate on site, adjustments happen during your visit instead of at a follow-up two weeks out.

What Your First Visit Involves

The first appointment is an evaluation: range of motion, strength, sensation, swelling and a careful history of how the injury happened and what it is stopping you from doing. From that we build a plan with a realistic timeline, and we will tell you honestly how long recovery is likely to take rather than what you would prefer to hear.

Working With Your Surgeon

A significant portion of our Smithtown caseload is post-operative, referred directly by hand surgeons across Suffolk and Nassau. When your surgeon specifies a protocol, we follow it precisely and report progress back. When something is not moving the way it should, we say so early — while there is still time to adjust.

Elbow and Shoulder Conditions

The name says hand therapy, but the specialty covers the whole upper extremity. Tennis and golfer's elbow, cubital tunnel syndrome at the inner elbow, rotator cuff rehabilitation and post-surgical shoulder care are all treated here. These are often connected — a wrist injury that changes how you lift will frequently produce elbow or shoulder symptoms within a few months, and treating only the obvious complaint tends to leave the cause in place.

Your Home Program

Most of your recovery happens between appointments. We give you a small number of specific exercises rather than a long sheet you will not do, show you exactly how each should feel, and revise them as you progress. If something is painful in a way it should not be, we want to hear about it before your next visit rather than after three more weeks of it.

Getting to the Smithtown Office

We are on East Main Street (Route 25 / Jericho Turnpike) in the heart of Smithtown, minutes from the Smithtown LIRR station and easily reached from Route 347, Route 25A and the Sunken Meadow Parkway. Patients coming from Kings Park, St. James, Nesconset and Head of the Harbor are typically here in under fifteen minutes. If our Hauppauge location at 2108 Joshuas Path is more convenient, we see patients there as well.

Insurance and Scheduling

Most insurance accepted — our team will assist with insurance verification and help coordinate your care.
